Everything you need to know about Laser Hair Removal

Spring has sprung and it’s time to think about swimsuits, shorts and shaving. In the summer months, I often see an increase in the number of patients experiencing painful ingrowns, folliculitis and general skin irritation as a result of shaving. For patients looking for a more permanent treatment option I often recommend laser hair removal.

The laser targets dark, pigmented hair in the active growth stage. Because there are three cycles of hair growth: active, transition and dormant, and since the laser is only effective on one cycle, laser hair removal requires a series of treatments usually spaced 4-6 weeks apart. After each treatment, patients will notice a patchy decrease in their hair growth.

Unlike waxing, hair does not have to be grown out to be treated, so you can shave or clip between treatments (however, no waxing or tweezing as this will disrupt the hair follicle). Six to nine treatments are usually required to produce the most noticeable results.

Beware that laser hair removal is not a permanent solution, there will always be some maintenance required as new hair follicles develop. Laser hair removal is also only effective on pigmented hair that the laser can target, no grays or blonde.

The Redness Bias: Does facial redness influence perceptions of people?

Facial redness, flushing, and bumps — also known as rosacea — is a very common problem.  It occurs in up to 10% of Caucasian individuals.  For those who experience redness it can not only impact them socially and emotionally but will often affect other people’s perceptions of them.

 

A recent study conducted by Dr Linda Papadopoulos in London demonstrated that among 6,831 individuals, photos of people who suffered from facial redness were rated to have poorer health and negative personality traits. Those same individuals, when shown with clear faces, were perceived as “well” and “healthy.”  Furthermore, participants were more likely to be friends with or hire the person shown in the image WITHOUT facial redness.

For the individuals who suffered from redness, 77% reported an emotional impact. They reported that other individuals have told them they drink too much or to recommend skin care interventions. However, these individuals reported much more control over their redness and symptoms once they were diagnosed. Once diagnosed with rosacea, 90% felt motivated to deal with their redness.

Winter Skin Care Tips

winter-skin-care-tips-minnesota-dermatologyWinter in Minnesota means lots of cold, dry air which reeks havoc on your skin. Dry skin can itch, flake, crack, and even bleed and without a change in your skin care, dry air can even make fine lines and wrinkles more noticeable.To help heal and prevent dry skin follow these few simple steps as provided by the American Academy of Dermatology.

1. Prevent baths and showers from making dry skin worse.

  • Close the bathroom door
  • Limit your time in the shower or bath to 5 or 10 minutes
  • Use warm rather than hot water
  • Wash with a gentle, fragrance-free cleanser
  • Apply enough cleanser to remove dirt and oil, but avoid using so much that you see a thick lather
  • Blot your skin gently dry with a towel
  • Slather on the moisturizer immediately after drying your skin

2. Apply moisturizer immediately after washing. Ointments, creams, and lotions (moisturizers) work by best-moisturizer-winter-skin-dermatology-mntrapping existing moisture in your skin. To trap this much-needed moisture, you need to apply a moisturizer within few minutes of drying your hands or body. We like to recommend Cetaphil and CeraVe as good everyday moisturizers for all of our patients.

3. Use an ointment or cream rather than a lotion. Ointments and creams are more effective and less irritating than lotions. Avoid scented products that can increase skin irritation. Shea butter and coconut oil also work well.

4. Wear lip balm. Choose a lip balm that feels good on your lips. Some healing lip balms can irritate your lips if they are scented. If you experience any tingling sensation, do not use the lip balm. We like to recommend Aquaphor to all of our patients.

5. Use only gentle, unscented skin care products. Some skin care products are too harsh for dry, sensitive skin. When your skin is dry, stop using:

  • Deodorant soaps
  • Skin care products that contain alcohol, fragrance, or alpha-hydroxy acid (AHA)

6. Wear gloves. Our hands are often the first place we notice dry skin. You can reduce dry, raw skin by wearing gloves. Be sure to put gloves on before you:

  • Go outdoors in winter
  • Perform tasks that require you to get your hands wet
  • Get chemicals, greases, and other substances on your hands
Wear cotton gloves under wool to avoid skin irritation and be sure to avoid wet gloves or cloths which can exacerbate eczema, cracking,  or cause sores.

7. Choose non-irritating clothes and laundry detergent. When our skin is dry even clothes and laundry detergent can be irritating. To avoid this:

  • Wear cotton or silk under your clothing made of wool or another material that feels rough
  • Use laundry detergent labeled “hypoallergenic”

8. Stay warm without cozying up to a fireplace or other heat source. Sitting in front of an open flame or other heat source can dry your skin. 

9. Add moisture to the air. Plug in a humidifier. If you can check your home heating system, find out if you have a humidifier on the system — and whether it’s working. Central heating actually blasts hot dry air throughout your home.

10. As always, continue to wear sunscreen on exposed areas of skin. Light actually reflects off the snow and can cause a severe sunburn. It is especially important to reapply sunscreen if you are performing physically extensive activities such as snowboarding or skiing when you can sweat off your SPF.
When to see a dermatologist
Your skin should start to feel better quickly. If these changes do not bring relief, you may want to contact the clinic. Very dry skin can require a prescription ointment or cream. Dry skin also can be a sign of a skin condition that needs treatment. A dermatologist can examine your skin and explain what can help reduce your discomfort.

 

Bed bugs at your hotel? A guide to prevention.

As we enter the holiday travel season, I would like to educate my Tareen Dermatology patients about bed bugs, and more importantly, how to avoid picking them up at hotels.

Bed bugs are parasitic insects of the cimicid family that feed exclusively on blood. Cimex lectularius is the most common type of bed bug and the type that causes most human infestations.

Unfortunately, bed bugs can live months (and even YEARS) between blood meals. Bed bugs also lay a great deal of eggs and have many offspring. In addition, bed bugs have evolved to become resistant to many insecticides and can survive weeks at extreme temperatures. Thus, PREVENTING bed bugs infestation is the best strategy.

At hotels:

  1. Look around: Bed bugs like right angles, so they can be found on headboards, bed frames, mattress piping, drawers, and windows. They can also be present on the under surfaces of furniture or in peeling paint.
  2. Check for droppings: Bed bug droppings typically are round brown splotches. Check mattresses and sheets as well as other furniture. If droppings are seen, bed bugs have been there and are probably still there.
  3. Follow the 3 foot rule: Check areas within 3 feet or so of hotel beds for evidence of bed bugs as this is the typical distance that they travel.

If you picked them up at your hotel and are now home:

  1. Utilize high heat: Heavy vacuuming and steaming may kill bed bugs if the temperatures are high enough.
  2. Lose the insecticide and forget cold: Insecticides are rarely effective. Bed bugs can also withstand freezing temperatures. Heat and mattress encasements tend to be most effective.
  3. Call Dr. Tareen: For resistant infestations, there are pills and topicals that can be prescribed.

If infestation with bed bugs does occur, do not worry. Bed bug bites are not dangerous. They are however very itchy and do look bad. So, follow the above tips to prevent these little nuisances from ruining your winter vacation.

Sunscreen and its ingredients are a controversial and personal topic. The media inundates us with mixed messages – at one end berating individuals for not frequently re-applying and on the other end claiming that sunscreen and its ingredients actually CAUSE cancer.

I always strive to be the voice of reason for my Minnesota patients. As a board certified dermatologist, I can assure my patients that NOT using sunscreen increases the risk of skin cancer. However, I agree that constant use of many chemicals MAY (and I stress may) be harmful.

Thus, the key ingredients that I stress for my patients are minerals, particularly zinc oxide. These mineral based sunscreens (which typically contain zinc oxide or titanium dioxide) sit on top of the skin as a shield and protectant from the sun. In contrast, the chemical based sunscreens (long names include avobenzone, octocrylene, ecamsule, etc.) must be absorbed into the skin to provide sun protection. Also good to know – the mineral sunscreens provide almost immediate protection while chemical sunscreens need to be applied approximately 30 minutes prior to going outside in order to be effective.

Zinc is the only ingredient that I use for my children’s sunscreen. In fact, the FDA recommends zinc as the main sunscreen ingredient for babies.

Zinc alone can be very chalky and white so it is important to look for the words “micronized.” This is a process whereby the mineral particles are made finer and more cosmetically elegant (but does not decrease the sunscreen efficacy).

A few brands that I like: Elta MD sunscreen, Vanicream sunscreen (made in Minnesota!), CeraVe AM, and of course the Tareen Dermatology SilkShade.

I hope this sheds light on a confusing topic and encourages more of my patients and friends to safely protect their skin with my favorite ingredient zinc!

 

Natural Treatment Strategies for Rosacea

Rosacea, a chronic vascular condition leading to redness, sensitivity, and broken blood vessels on the face, is very common in Minnesota. Here are a few natural ingredients to look for in skin care products to help manage your rosacea 
1. Niacinamide:  This is a B vitamin that decreases inflammation. Oral niacinamide dosed at 750 mg a day with a copper and zinc supplement was shown in a study to benefit 75% of users at 8 weeks. Topical niacinimide has also been shown to reduce the burning associated with rosacea.
2. Licorice: Licochalcone A (licorice extract) has been shown to help improve the stinging, tingling, and tightness associated with rosacea. Studies have shown that it takes at least 8 weeks for licorice extract to show an improvement.
3. Green tea: Catechins are the main anti-inflammatory ingredient in topical green tea. In studies, both oral and topical green tea have been shown to decrease the inflammation associated with rosacea.

The Best Way to Shave

Many men are prone to ingrown hairs. The scientific term for this condition is called “pseudofolliculitis barbae.”

Ingrown hairs in the beard area result from small, shaved hairs curling back into the skin. The embedded hairs then form a bump. These bumps can get inflamed, infected, and eventually lead to scarring.
The easiest treatment is to stop shaving. According to experts, ingrown hairs spontaneously release after 1 centimeter of growth. Dr. Tareen and her colleagues frequently write letters of medical necessity for patients’ jobs that document their necessity to limit shaving.
Some men may desire or need a clean shaven appearance. Below are great tips to help prevent ingrown hairs:
1. Use a single- blade razor:  Single blade razors shave less closely and prevent small hairs from curling into the skin. Razors should be replaced every five to seven shaves.
2. Use electric clippers: Clippers are another great option. The blade setting that allows 0.5-1mm of stubble tends to be the most effective at limiting ingrowns.
3. Shaving technique is important:  Before shaving, men should wash with a mild cleanser using a gentle circular technique to free any embedded hairs. During shaving, a moisturizing shave cream should be used. Shaving should always be done in the direction of hair growth.
4. Laser hair removal is a great option: Permanent reduction of hair is both cost and time effective when factoring in the cost/ time of shaving.

Probiotics help prevent acne breakouts

At Tareen Dermatology, we utilize a combination of holistic and medical approaches to achieve clear skin for our patients. Recent research demonstrates that diet may have a significant impact upon acne production. Here is the scoop:

What food to AVOID if you are acne prone:

1. High glycemic index foods: These include foods such as white bread, white potatoes, crackers, pretzels, white rice and other processed, carbohydrate heavy foods. In a 10 week study of teenagers with acne, biopsies of the skin were taken before and 10 weeks after a low glycemic diet. During the 10 weeks, the teens instead consumed multigrains, vegetables, and protein. All of the teenagers’ acne improved. Skin biopsies demonstrated less oil production and overall less inflammation.

2. Skim milk: Skim milk increases the amount of insulin produced by the body. Insulin stimulates acne production and in some women who are prone to excess of testosterone, also facial hair. Cheese and yogurt have not been shown to stimulate acne.

What foods to EAT if you are acne prone:

Yogurt and probiotics: Yogurt with live and active cultures and probiotics actually help protect against acne production. Probiotics regulate the amount of “good bacteria” in the digestive tract and reduce levels of systemic inflammation. Studies have shown that probiotic consumption can also reduce formation of eczema (an added bonus!).
